Prison combination toilet bubblers serve double duty as a sink faucet and drinking fountain — which is why they must be installed with the outlet facing up. A repeat offender of TICI violations had to completely uninstall every fixture to flip one screw on the back of each bubbler.

When a wet rotor circulator pump failed to operate on a radiant floor heating project, the culprit was simple: the pump had been installed with the motor shaft vertical because the installer thought it looked better. All wet rotor pumps must be installed with the shaft horizontal, as the bearings aren't designed for thrust loads.

Prison flush valves that wouldn't work properly pointed to a water pressure problem. The cause was buried underground — a hot tap connection where the installer had only drilled the small pilot hole instead of the full-diameter cutter hole, restricting the water supply to a quarter-inch opening.
